In a patient who the family reports isn't breathing write, and a patient who says I can't breath well on this tying screen since he can't speak 2nd to ALS Lou Gerigh's Disease and isn't trached yet, what tests to do? Chest xray, blood work? Ct angio chest? Increased work of breathing is from what? Something plugging or compressing the lungs the parts that need ventilation like edema or mucus or aspiration or pleural effusion or pneumothorax. Or lack of perfusion from lack of blood flow to the lungs despite open airways? Or lack of strength in the muscles of respiration? Then figuring out if the patient needs intubation? Dnr/dni/DNR-CCA status to be clarified. The first part requires a cxr or a bedside pocus ultrasound lung scan or a bronchoscopy to confirm. 2nd part usually requires a Ct angio lungs but a pocus ultrasound heart exam would be prefered. 3rd part hmmm. Abg? Vbg? Nifs? MIP? Hmmm. Acute Respiratory Failure from hypoxia and/or hypercapnea. Learning lessons.
