At GEM, over 950 minimally invasive esophagectomies have been performed between 1997 and 2018, spanning transhiatal, McKeown, and Ivor-Lewis approaches. He emphasizes tailoring the surgical approach to both tumor location and histology. For mid-third esophageal cancers, transthoracic McKeown esophagectomy remains standard. For lower-third adenocarcinomas, the Ivor-Lewis approach is preferred, while selected squamous cell carcinomas in the lower esophagus may be treated with transhiatal esophagectomy due to the concept of field cancerization.
