Clinical Trial: SBRT vs IMRT For Low Risk to Intermediate-Risk Prostate Cancer | PCRI

PCRI's Executive Director, Mark Scholz, MD, explains a clinical trial (NCT03367702) for men with High-Sky and Low to Basic-Teal (unfavorable low-risk to favorable intermediate-risk) comparing the cure rates and side effect profiles of stereotactic body radiation (SBRT) and intensity-modulated radiation (IMRT). 0:09 The purpose of this clinical trial is to compare the cure rates and side effect profiles of SBRT and IMRT for intermediate-risk patients. 0:28 SBRT and IMRT are both forms of beam radiation, but SBRT is given in less than two weeks and IMRT is given over five weeks. In the past, IMRT was given over nine weeks, but previous clinical trials found that five weeks was sufficient. 1:00 This clinical trial will involve follow up until 2030 and most patients will be cured whether they are given SBRT or IMRT. Oftentimes side effects with radiation are delayed by years, and so follow up until 2030 will allow investigators to properly compare the side effect profiles of the two forms of treatment. 2:01 This trial is designed for men with High-Sky and Low and Basic-Teal. That means a Gleason score of 6 and a PSA over 10 or a Gleason 3+4 and a PSA under 10 and no biopsy cores more than 50% involved. Since the patients in this study have such favorable situations, no androgen deprivation therapy is needed. The beam radiation will be the only form of treatment. 2:40 Men in High-Sky and Low-Teal should be aware that active surveillance may be an option and that radiation may be overtreatment. 3:05 In the past, there were concerns that the two-week protocol of SBRT could lead to a higher incidence of rectal burns; however, the advent SpaceOAR has reduced those concerns dramatically. four and five-year follow-ups on SBRT patients who used SpaceOAR have not found any evidence of rectal burns. 3:52 All the patients in this trial will be given an MRI to see if they are helpful in predicting future relapse rates. 4:17 No one with a prostate larger than 60cc is allowed to participate in the trial because they are at a higher than average risk of developing side effects from the increased radiation required to treat the larger gland. This makes it harder to compare the side effect rates of the two different forms of radiation. 5:02 If you do not want to participate in this trial, then both treatments are good options for treatment.
