Open Tip Rhinoplasty Approach Explained | Dr. Karishma Kagodu | Bengaluru

Open rhinoplasty surgery is a procedure in which the incision used to access the structures of the nose results in broad exposure of the underlying cartilage and bone. Typically, a small incision is made across the underside of the nose that is then connected to incisions made within both nostrils. The skin overlying the nose is reflected up and off of the structural framework of the nose. This allows the surgeon to have broad visibility of all the components of the nose. Before beginning the incision a series of photographs of the patient is first shot to understand the patient needs. The photographs are shot in different angles (Frontal, Side, Oblique, and Basal views) for accurate addressal of concerns such as Nose Size, Nose Shape, Nose Angle, and Nose Bump. Patient's general health and fitness is taken into consideration to avoid any future issues before entering the OT. A pre-procedure marking is done in the sterile area to be operated/ corrected right after the patient is administered with General Anesthesia. The patient had concerns of boxy or a broad nose tip. During an open Rhinoplasty: An incision will be made just beneath the tip of the nose. The nose will then be retracted backwards, dissecting the nose cartilage with care being taken to not incise the cartilage that separates the nostrils. The flap will be raised off of the nose for maximum exposure and work can now be done to reshape the nasal tip, bridge, nostrils, or septum repair. In this particular patient the nasal lower lateral cartilage was broad. After precise measurement, the cartilage has been reduced to 6mm to suit the facial profile. To create a tripod shape at the tip of the nose, the cartilages are stitched in the midline between the nostrils and brought closer together to make the nose look slimmer and sharper. Once completed, the flap will be re-draped over the bone framework and external contouring and shaping is completed. The incision will be closed with sutures and a splint or cast will be placed over the nose. The objective of open rhinoplasty is to achieve a facial harmony by changing it to required shape, size, and angle of the nose. Watch the video for the process.
