Otosclerosis, as the name implies, ‘oto’ and ‘sclerosis’ mean that there is a sclerosis or a loss of something in the oto. ‘Oto’ means the ear. This essentially means that here is a disease process which affects the ear, primarily in which one of the bones of the ear, the smallest bone in the body called as ‘stapes’, it stops moving. The essential part of hearing is that all the three bones of the ear should be moving as freely as possible so that any sound which is perceived in the drum is amplified 22 times, because of the hearing mechanism and that is presented to the ear. When this amplification doesn’t happen, like in the case of otosclerosis, then you have hearing loss. So otosclerosis, is a common cause of hearing loss and it can be easily diagnosed. One you have to do an audiometry examination. You can do an ototscopy examination to see the drum is intact. One of the commonest cause of conductive hearing loss is otosclerosis, provided there is no fluid behind the drum. So we do an otoscopic examination. We do something called as the audiometry examination, which will prove a conductive hearing loss and there is something called as the impedence or the tympanometry, where you get a curve called as the AS type of curve. So it is a fairly simple diagnosis. You obviously take a scan of the temporal bone also to rule out any other factor. The definitive diagnosis of otosclerosisis done on the operating table and otosclerosis is a relatively simple disease to manage. What we usually do is something called as the stapedectomy surgery where the stapes, the bone which is fixed is removed within the drum, without any perforation and it is replaced by a Teflon prosthesis. Properly done stapedius surgery without any comorbid condition, usually gives very good results. So you have hearing loss of any etiology, meet your otologist as soon as possible.
