Diastematomyelia is a congenital spinal condition consisting of a longitudinal split in the cord separated by a bony or cartilaginous septum. They account for 5-6% of all congenital spinal defects. Majority of such patients are symptomatic with varied presentation consisting of leg weakness, low back pain, scoliosis, incontinence. There are 2 types of Diastematomyelia. Type 1 is characterised by a duplicated dural sac separated by a bony or cartilaginous septum and majority of patients are symptomatic. In type 2, there is a single dural sac with 2 hemicords separated by a thin fibrous or fibrovascular septum. Most of these type 2 patients are mildy symptomatic or asymptomatic. In this case, we have demonstrated the spur splitting the cord and dural sac into two and we have excised the spur completely and the 2 dural sacs have been converted into a single sac with division of the filum terminale to complete free the spinal cord from any kind of tethering.
