Smaller incisions mean less consistent drainage and certainly removal of less membrane material, be it infected/inflamed or not. Also, because the incision is smaller, it is more likely to re-accumulate overnight so you may need to redrain it the following day. Usually you can open these back up just with a little pressure but if the patient isn't seen until day 2 or 3, these can reseal and require re-incision altogether.
