In this case presentation, Dr. Khaled Moumneh discusses the complex clinical course of a 58-year-old man who developed a macular fold following retinal detachment repair. Initially treated with pars plana vitrectomy, laser, and gas tamponade, the patient experienced a series of complications including recurrent detachment, a macular hole, and chronic cystoid macular edema requiring repeated Ozurdex injections. The lecture focuses on the pathophysiology, diagnosis, and classification of macular folds—a rare but visually significant complication occurring in approximately 3% of rhegmatogenous RD repairs. Using multimodal imaging, the case highlights key OCT findings and differentiates between ripple, taco, and displacement folds. Dr. Moumneh outlines both conservative and surgical treatment strategies, emphasizing the importance of SRF management and individualized surgical approaches for persistent, vision-threatening folds. This is an important teaching case for retina specialists managing complex post-operative complications.
