Wheezing is always an abnormal breath sound that occurs when there are vibrations of the airway wall that are caused when air flows at a high velocity through a narrowed airway. This is referred to as bronchoconstriction. When air rapidly flows through an obstructed airway, it creates a high-pitch sound that usually occurs on expiration, and that is what we refer to as wheezing. ➡️ Causes of Bronchoconstriction: ▪ Bronchospasm ▪ Mucosal edema ▪ Inflammation ▪ Tumors ▪ Foreign bodies ▪ Pulmonary Edema ➡️ Example: Let's say you're a Respiratory Therapist and you've been ordered to give a bronchodilator breathing treatment. Before the treatment, the patient is wheezing and the wheezes are very high-pitched and are present during the entire expiratory time. Then, after the treatment, you notice that the sounds become lower in pitch and they occur only late in the expiration. At this point, you notice that the pitch and duration of the wheeze have both decreased. This change indicates that the patient has improved. ➡️ TMC Exam Tips About Wheezing: Wheezing can be caused by bronchospasm, and when this is the case, you should recommend a bronchodilator like albuterol. You may see a question where a patient has a unilateral wheeze which indicates that a foreign body obstruction is present. -- Bilateral wheezing - recommend a bronchodilator. -- Unilateral wheezing - recommend a bronchoscopy. An example of a foreign body obstruction is a bronchial mass as you would see in a patient with lung cancer. Or, in children, it could be that they swallowed an object that is causing unilateral wheezing. ➡️ One More Example for the TMC Exam: Let's say you're assessing a patient in respiratory distress and they have diminished breath sounds. You don't hear any wheezing but the patient isn't moving much air. So you proceed to give the treatment. After the treatment was given, when you auscultate the patient again but this time you can hear audible wheezes. ➡️What Diseases Can Causes Wheezes? Bilateral wheezes are caused by bronchoconstriction which often means that the patient has an obstructive disease. ➡️ CBABE ▪ C - Cystic Fibrosis ▪ B - Bronchiectasis ▪ A - Asthma ▪ B - Chronic Bronchitis ▪ E - Emphysema Now let's talk specifically about Expiratory Wheezing and what is the difference between monophonic and polyphonic wheezes? Expiratory wheezes indicate that there is an obstruction of intrathoracic airways such as occurs with lung diseases like bronchitis and asthma. ➡️ Monophonic vs Polyphonic Wheezes A monophonic wheeze indicates that a single airway is partially obstructed. Monophonic wheezing may be heard during inhalation and exhalation or during exhalation only.
