Inferior alveolar nerve block (IANB) failure is a common clinical problem in dentistry, especially in cases of symptomatic irreversible pulpitis and highly inflamed mandibular teeth. In this video, I explain a chairside protocol for managing mandibular anesthesia failure and improving success with the inferior alveolar nerve block. We discuss practical strategies used in endodontic practice when the mandibular block does not provide adequate pulpal anesthesia, including: • Pre-operative NSAIDs for symptomatic irreversible pulpitis • Increasing anesthetic volume within safe dosage limits • Proper injection height for the inferior alveolar nerve block • Allowing adequate waiting time for mandibular anesthesia • Supplemental buccal and lingual infiltrations • Using articaine for mandibular infiltration • Periodontal ligament (PDL) injections • Intraosseous anesthesia techniques • Intrapulpal anesthesia when a small portion of pulp remains sensitive These techniques help improve success rates when managing difficult mandibular anesthesia cases. #MandibularBlock #DentalAnesthesia #InferiorAlveolarNerveBlock #Endodontics #Dentistry
