In microincision vitrectomy surgery, depending on the case, often multiple instrument exchanges are required. Occasionally you require fast instrument exchanges. Under dim light or dark room, you cannot identify the port entry. In this PDR case with bleeding, we required a fast cautery exchange to cauterize the bleed; however, we struggled to insert it fast enough. Dimming the room lights and switching the microscope lights off creates an optimum visual environment for vitrectomy. However, the microscope illumination needs to be constantly toggled on and off for an instrument exchange. A constant illumination is not possible, and the microscope light needs to be repeatedly switched on and off for an instrument exchange. We aim to develop a cost-effective device that provides sufficient illumination for instrument exchange during vitrectomy surgery without constantly switching the microscope light on and off. We introduce to you the Glowport. This is the Glowport, a 3D-printed phosphorescent glow device. This is compactly fitted over a 25-gauge trocar cannula as shown. After placing the infusion cannula, both the superior cannulas are placed with the glow device on it. The glow device is then exposed to light from the microscope and the endo-illuminator. Under mesopic and dim room light conditions, we visualize the Glowport glowing in the dark. The Glowport, with its property of phosphorescence or glow in the dark, helps us in smooth and efficient instrument exchanges during the surgery without having to repeatedly switch on and off the microscope light. The glow stays for 10 to 15 minutes and it regains the glow after a brief exposure to the endo-illuminator light. Ocular touch errors as well as effective surgical time are reduced for novice as well as experienced VR surgeons. We designed a phosphorescent glow-in-the-dark trocar cannula for smoother instrument exchange with the device being used in real-time mesopic conditions. In conclusion, the Glowport is an economical and innovative tool crafted from 3D-printed materials that provides essential illumination for smooth instrument exchanges during vitreoretinal surgery. This device helps in reducing the procedure time and is advantageous for both novice and experienced VR surgeons.
