There are two groups of DOAC drugs. One group binds to and inhibit Thrombin The only drug available for clinical use in this group is Dabigatran. This was first DOAC drug that was used clinically. Dabigatran reversibly binds to thrombin and inhibit its action that inhibit. Thrombin then is not available for some of its main functions that is conversion of fibrinogen to fibrin and activation of other clotting factors. Dabigatran has immediate and predictable anticoagulant effects, so adjustment of dose and routine monitoring of Ac effects of Dabigatran is not required. However, in certain situations we need to monitor or assess anticoagulant effects and in that case we do not have routinely available tests that can reliably measure AC effects of Dabigatran. DOAC drugs have now become most commonly used oral anticoagulant drugs and with their increasing use we will learn more details about monitoring and adjustment of dose of these drugs.
