So this is not actually a disease per se. It is a disorders where more blood flows into the anal cushions. So they can present with bleeding or is a swelling. So depending in the stage, we call it as a first degree, second degree, third degree. Depending on the stage, there are different varieties of treatment available. Amongst them laser is one of them and for open surgery is the other one. There is something called as Doppler guided hemorroidal artery ligation. There is something called as rubber band ligation, something called as infrared coagulation. There are varieties of treatments available depending on the stage. Lasers have their own limitation and advantages. So where do you use lasers and at what stage, where you may have to use? If lasers are not available, even with Open Hemorrhoidectomy, what we call as Hemorrhoidectomy gives equally good results. What happens is with Open Hemorrhoidectomy will be more painful, whereas for the first and second degree Hemorrhoids, we need not use lasers, we need to do Open Hemorrhoidectomy or open surgery we can use simple procedures like a rubber band ligation or infrared coagulation, injection sclerotherapy, much simpler procedures than lasers and then third and fourth degree Hemorrhoids, where they are much big, we can use lasers or Doppler, where using Doppler, we can close the supply of the blood vessels and shrink the hemorrhoidal masses and that itself may be useful for a particular degree of hemorrhoids or if required we use recto anal repair, where we put a few stitches to reduce the size of it. So varieties of procedure are available depending on the type o f hemorrhoids. If it is highly bleeding hemorrhoids, we do it differently, if it is a mass protruding out we do it differently. So mass hemorrhoidoplasty take s care of all the degrees of hemorrhoids. So laser should not be hyped up as treatment procedure for hemorrhoids or nay other disease. Laser is just one armamentarium for the treatment of hemorrhoids. So you can use laser depending on the need. So it is not that laser hemorrhoidoplasty or laser fistulotomy or fistulectomy. Laser is one gadget. The way we do it , it will give you good result and open hemorrhoidectomy, where there is no availability of this, it should not be condemned, but only issue with open hemorrhoidectomy is that it can cause a lot of pain and that can be minimized with minimally invasive hemorrhoidoplasty.
