83 year-old male with a history of AMD experienced a recent decline in vision in his left eye. Fundus photo of the left eye showed a large subretinal hemorrhage in the macula. OCT showed subretinal hemorrhage with peaked PEDs. FA demonstrated leakage, and ICGA demonstrated polyps at the level of the choriocapillaris, which are characteristic for polypoidal choroidal vasculopathy (PCV). The patient was treated with photodynamic therapy and was subsequently lost to follow up so response to treatment could not be ascertained. PCV is an abnormal development of polypoidal choroidal vasculature which tends to hemorrhage and leak. It does not respond to anti-VEGF as well as typical nAMD, however, it appears that combination therapy with anti-VEGF and photodynamic therapy results in a better treatment response.
