Ear infections it can be classified and infections affecting the canal the inner ear and the middle ear. Canal infections are usually skin level infections which can be managed with medications. Middle ear infections when they are acute, when a doctor uses a term ‘acute' it means anything of a short duration of time, say 4 weeks' time is called as acute, anybody who has an acute problem generally does well with medical treatment and keeping the ear dry by not allowing water to get into the ear or using ear buds or anything else and generally the by takes care and heals on its own. The ear is restored to its normalcy. Surgery as an option is only available for those patients in whom there are repeated infections of the middle ear this condition is called as chronic otitis media. Chronic otitis media means the patient generally has some amount of hearing loss. They will feel that the ear is blocked or they will have some amount of ear discharge happening from the ear. That means some amount pus like material or foul smelling discharge or wetness in the ear and they see something wet discharge in the ear, some wetness in the ear and when they bring it closer to their nose, it smells really bad. So these are the kind of symptoms which the patient will have when they haver chronic otitis media. Chronic otitis media, there are two types of entities tubotympanic type or the relatively safe type of otitis media and another entity called as the cholesteatoma. Of somebody has cholesteatoma. Then we are very aggressive and requires surgery because cholesteatoma can cause lot of problems by eroding bone. Chronic otitis media, which have been there for a very long time, for more than 3 months of time, which is not healing with a hole in the eardrum, this will also require surgery, mainly because after 3 months or so the chances of eardrum healing in its own are removed. There are occasional questions on whom it will heal on its own. But in majority of them the hole will be persisting for many years as long as the hole remains other is a potential for the ear to get infected again and again, which can lead to hearing loss and other problems. Hence chronic otitis media is largely a surgical problem it will require a surgery for restoration of the normalcy of the ear and to prevent any deterioration of the hearing.
