For advanced epithelial ovarian cancer, achieving "optimal cyto-reduction" (also called debulking) – surgically removing all visible tumor, ideally to "no residual disease" (NRD) or less than 1 centimeter of residual tumor – is one of the most significant prognostic factors for survival. This complex surgery should ideally be performed by a specialist Gynecologic Oncologist who has the expertise to perform extensive procedures like bowel resection, splenectomy, or diaphragm stripping if needed to achieve optimal debulking.
