Laminitis is a very complex condition that I won't try to cover fully here. Keeping it as simple as possible, I'll just cover some of the basics. Laminitis is an extremely serious condition that every horse owner fears but quite often fails to understand. Most people tend to start worrying when things get really bad, usually when the horse is going through an episode of ACUTE laminitis. In that case, the poor horse is usually in terrible pain, is trying to "unload" his hooves and can barely walk. That's when the vet is called and that huge changes in the diet, the exercise and quite often the trimming have to be made in order to save the horse. What is happening to the horse at that stage is that the lamina (= the glue holding his nail to the rest of the foot), is inflamed A LOT and the nail, especially if it is long, is detaching from the bone, causing extremely serious damage to the hoof on top of excruciating pain. This process affecting the hooves, doesn't however originate in the hooves themselves but higher up in the body and is usually connected mostly to an overload of carbohydrates in the diet and a lack of exercise. It is roughly the equivalent of type II diabetes for humans, a lifestyle disease that can affect the extremities ( toes, fingers, etc. ). CHRONIC laminitis is exactly the same problem, with the same causes and the same solutions. It is just happening more slowly and little by little. So it is extremely often ignored or overlooked until things get way worse. It should nevertheless be addressed right away with the same seriousness than the acute phase. To keep it as simple as possible : you should start worrying about laminitis ( sometimes a death sentence for the horse ) as soon as your horse is overweight and / or as soon as you see that his nail looks like it is growing away from the rest of the foot. Finally, when you hear about "rotation", be aware that it can be 2 different things. If there is BONE rotation, P3 is detaching from P2. You need X-rays to confirm it and that is a very grave situation that is going to be extremely difficult to solve. In most cases of bone rotation, the horse is euthanized. HOOF CAPSULE rotation is "simply" the nail detaching from the rest of the foot, and basically, from the bone. It is sadly a very common thing but it can usually be reversed by addressing the diet (avoiding sugars and starch at all costs), the exercise ( the horse has to move as much as possible ) and the trimming (keeping the nail as short as possible so it is not loaded and "pushed" away from the rest of the foot.)
