Thyroglossal Duct Cyst: Key Ultrasound & Color Doppler Findings with Prognosis Explained

A thyroglossal duct cyst (TGDC) is a congenital neck anomaly resulting from incomplete regression of the thyroglossal duct during thyroid gland development. It is the most common midline neck mass, often presenting in children or young adults. TGDCs typically appear as painless, mobile swellings that move with swallowing or tongue protrusion, though infection or inflammation may cause pain. On ultrasound, TGDCs appear as well-defined cystic lesions. In this case, the cyst is midline, 2 cm above the thyroid cartilage, with thin walls, two internal septae, and mildly echogenic contents, likely colloid or debris. Mild peripheral vascularity is noted without internal vascularity, consistent with a benign lesion. Surgical removal via the Sistrunk procedure is curative, offering an excellent prognosis and low recurrence risk.
