American Airlines 191 was a commercial passenger flight from Chicago, Illinois to Los Angeles, California on May 25th, 1979 carrying 271 people. Crash: As the plane started to rotate the No.1 engine separated taking a large section of the wing's leading edge with it, the engine also cut a hydraulic line to the wing causing a severe left bank, the plane crashed into a trailer park killing everyone on board and 2 people on the ground. To this day it is America's deadliest plane crash. Investigation: Improper maintenance procedures on the engines of American Airlines and Continental Airlines DC-10's was the main culprit, they mounted the engines on forklifts and jammed them back into place under the wing leaving them unsecured and causing corrosion on the engine mounting. British Midland 92 was a commercial passenger flight from London, the United Kingdom to Belfast, the United Kingdom on January 8th, 1989 carrying 126 people. Crash: While the plane was climbing through 28,000 feet a fan blade on the left engine separated, the Captain misidentified the problem as the right engine failing so he shut it down, the crew diverted to East Midlands airport. The Captain increased the speed and this caused the left engine to fail completely, the crew tried to restart the right engine but they couldn't due to the low airspeed so they tried to glide into the airport, they failed and the plane crashed into an embankment just past the M1 motorway 630 meters short of the runway killing 47 people with 79 surviving. Investigation: The engines on the 737-400 were just an upgraded version of a previous engine and above 10,000 feet the engines tended to vibrate, they were unproven and only tested in a lab and the constant vibrating caused the structure of the fan blades to deteriorate quickly and eventually fail. Helios Airways 522 was a commercial passenger flight from Larnaca, Cyprus to Prague, Czechia with a stopover in Athens, Greece on August 14th, 2005 carrying 121 people. Crash: At 9:12 AM when the plane was at 12,000 feet the Cabin Altitude Warning sounded indicating low cabin pressure, the pilots thought this was the Take-off Warning System alarm and ignored it, the crew reported an Air Conditioning problem at 29,000 feet and a Maintenance worker asked them if the pressurization panel was set to AUTO. The crew never responded and the plane climbed to 34,000 feet, the plane then entered a circling pattern over Athens before the engines flamed out, the remaining crew member was unable to get ahold of the plane and it crashed into a hill killing everyone on board. Investigation: When the plane was in London earlier that day the crew requested an inspection of the aft right maintenance door, the worker set the pressurization system to Manuel and didn't set it back to Auto meaning the crew had to manually pressurize the plane, the crew didn't know this and didn't pressurize the plane. Atlantic Airways 670 was a commercial passenger flight from Sola, Norway to Årø, Norway with a stopover in Sørstokken, Norway on October 10th, 2006 carrying 16 people. Crash: As the plane landed in Sørstokken the pilots were unable to stop the plane, they applied full brakes but it was no use and the aircraft overran the runway and over a steep ledge falling into trees and catching on fire, 4 out of the 16 people died. Investigation: The pilots activated the emergency braking system as they believed normal breaking wouldn't stop the plane in time. The emergency braking system caused the tires to heat up and create a layer of steam between the ground and the tires, this made the brakes ineffective and the plane overran the runway. Birgenair 301 was a commercial passenger flight from Puerto Plata, the Dominican Republic to Frankfurt, Germany with stopovers in Gander, Newfoundland, and Berlin, Germany on February 6th, 1996 carrying 189 people. Crash: While the plane was climbing out of Puerto Plata it suddenly stalled at 6,000 feet, the crew were unable to regain control and the plane began to dive, the crew leveled it out but too much altitude was lost and the plane crashed into the ocean killing everyone on board. Investigation: A wasp had made its nest inside one of the plane's pitot tubes giving the speedometer incorrect information, the pilots thought the malfunctioning pitot tube was correct and they trusted it as they crashed. Pakistan International 268 was a commercial passenger flight from Jinnah, Pakistan to Kathmandu, Nepal on September 28th, 1992 carrying 167 people. Crash: While the plane was descending to Kathmandu it crashed into a mountain in a cloud at 7,300 feet killing everyone instantly, the plane disintegrated and the wreckage fell to the base of the mountain. Investigation: The crew had non-pertinent conversations during the descent and had begun descending early, they had flown 1000 feet underneath their assigned altitude and the pilots weren't paying attention until impact.
