There's an open rhinoplasty and a closed rhinoplasty. Open rhinoplasty refers to making an incision underneath the nose, known as the columellar incision, and a closed rhinoplasty doesn't involve that incision. Open and closed rhinoplasty are for very different things. Closed rhinoplasty is good for people that seek alterations that are minimal, involving the middle and upper third of their nose. Open rhinoplasty is good for people seeking those kind of changes, in conjunction with other types of changes to the tip of the nose. Depending on the approach, open versus closed rhinoplasty, the scarring is going to be a lot different. If you have an open rhinoplasty, then you'll have what we call a columellar incision, which is an incision beneath the nose. Typically, that heals really well and you can't see it six months after the surgery. Now, if you have a closed rhinoplasty, the incisions are hidden on the inside of the nose. It's still really important to take care of those incisions, even though you can't see them. What you want to do is also keep them nice and moist with a ointment, and also to use salt water irrigations that we provide to also keep that crust and clot out of the nose. It's really important to see a surgeon that's well versed and trained in both approaches, closed and open rhinoplasty, so that they could sit down and understand what it is that you need and the best possible approach for you.
