I want to take a little bit of time to talk to you guys a little about thoracic endometriosis. So thoracic endometriosis is basically endometriosis that goes up to the upper abdomen, including the diaphragm, the lung space, or the heart. This can occur about 1 to 2 percent of patients with endometriosis. This actually might be an underestimation because non specialists or even some OBGYN surgeons will refuse or don't bother to look in the upper abdomen to evaluate for thoracic endometriosis. Clinically, thoracic endometriosis can be a little bit hard to diagnose. The reason being is when you get inflammation in the pelvis, inside your belly, things move in a clockwise fashion. So if you have inflammation inside your pelvis, it will actually travel up to the upper right abdomen. So you can get some of the symptoms that are concerning for thoracic endometriosis, like difficulty breathing, rib pain, muscle pain in the upper abdomen. It can also be a little bit more obvious to diagnose when you get things like catemanial pneumothoraces once a month or if you have hemoptysis when you have your period. Those are almost telltale signs of endometriosis as spread to the lung cavity or the pleural space. So with surgery, you should make sure your surgeon at least feels comfortable or will evaluate for upper abdominal and thoracic endometriosis. A lot of times a specialist, especially an expert endometriosis surgeon, can excise endometriosis off the diaphragm rather safely. Sometimes the thoracic endometriosis can be a little bit deeper. It can actually invade through the diaphragm into the plural space. Sometimes this does involve a diaphragmatic resection of the muscle, whether it be whole or partial. And if you get to some of those levels, a lot of times you're going to want your surgeon to be working with a skilled cardiothoracic surgeon that is also familiar with endometriosis and what is necessary to treat endometriosis, especially if you start having some of the more thoracic symptoms like the hemoptysis or ketamine pneumothoraces, you really want that to be done with a trained specialist that has a good cardiothoracic surgeon that they can work with. Overall thoracic endometriosis can be difficult and tricky to treat, but with the right and skilled hands, it can be very treatable and help with 100 percent excision with these surgeries.
