The tracheal bronchus, also known as the suis bronchus or ‘pig bronchus,’ is an anatomical variation in which the right upper lobe bronchus originates directly from the trachea above the carina, rather than from the main bronchus. This anomaly, more commonly found in pigs, occurs in a small percentage of humans, leading to the nickname ‘pig bronchus.’ Typically, this variant is asymptomatic and often discovered incidentally during imaging. In these scans, we can see the tracheal bronchus marked, branching off from the trachea rather than following the usual path through the right main bronchus. The carina, which normally serves as the tracheal bifurcation into the main bronchi, is located just below this branching point. While the tracheal bronchus usually doesn’t cause any issues, it’s important for clinicians to recognize it, especially in cases requiring intubation or bronchoscopy. In rare instances, this anomaly may be linked to tracheal stenosis or infections due to altered airway drainage. An understanding of this anatomical variation helps avoid complications during respiratory interventions and enhances diagnostic accuracy in imaging.
