Modified Inverted ILM Flap Technique for Macular Hole After Vitrectomy for Retinal Detachment

The incidence of macular hole following surgery for retinal detachment has been reported to be between 0.24% and 1.9%. I decided to perform a modified inverted ILM flap technique. Key points: -The first thing I do is conserve the flap around the macular hole. -It's important to make sure that the flap is attached to the edge of the macular hole only. -Using a low vacuum (10-15) and low cut rate (100), I trim the flap to the size of the macular hole. -During the fluid air exchange, you can reposition the flap by decreasing the intraocular pressure (15) and aspirating the liquid near the fovea in the direction you want the flap. -REMEMBER NEVER TOUCH THE MACULAR HOLE. -DO NOT TRY TO INSERT THE FLAP OR MASSAGE THE EDGES OF THE MACULAR HOLE.
