So why does gynecomastia occur? In general, considering males’ life, there are 3 stages in which gynecomastia is observed. One of them is the growth of the baby's breast due to hormones passed through the mother in the newborn period One of them is the growth of the baby's breast due to hormones passed through the mother in the newborn period, the second one is the growth of the breast due to hormonal changes during adolescence, and the third one is the growth of the breasts due to the decrease in testosterone in later ages. But the most common cause of gynecomastia today is due to excessive weight gain. These patients usually have a growth in breast fat, which is called pseudogynecomastia. Although there may be a growth in breast tissue in some of these patients, the weight is the excess fatty tissue and with gynecomastia, fat occurs in the abdomen, sides of abdomen, and lower back. Is there any age limit for gynecomastia surgeries? As with any aesthetic operation, we usually want the patient to be 18 years old for gynecomastia surgery. We do not prefer to perform surgery at an age below this. There is another condition specific to gynecomastia. In order for a gynecomastia to be operated, it must be permanent for more than one year, the hormonal values of the patient must be evaluated, and there must be no increase in these hormonal values, and the surgery must be determined by performing an ultrasound, and what the content of breast growth is before the surgery. So this content can be breast tissue, it can be just a fatty tissue, or breast tissue, or both. The gold standard in gynecomastia is vascular liposuction. In this surgery, the fatty tissue in the breast are first melted, then the melted fats in the breast are aspirated with cannulae and removed from there. After this surgery, if the patient does not have any excess skin or if the excess skin is not large, no excess skin occurs after the surgery. But if the growth in the patient's breast is too much, it is absolutely necessary to remove that excess skin while conducting gynecomastia in these patients with vaser liposuction. In order to remove excess skin, we usually prefer to make an incision from the nipple and another to surround it with a larger incision, remove the excess skin by removing the tissue and shrinking the excess skin. As I just said, vaser liposuction is the gold standard in gynecomastia, and the first indication that the vaser received approval for use is gynecomastia surgery. The most important reason for the use of vaser in gynecomastia can be effective on both fats and if it has a soft consistency, it can break down breast tissue, that is, both breast tissue and fatty tissue can be removed from the breast and surgery can be completed at the same time with the help of cannulae. Since gynecomastia in men usually occurs with excess fat, fatty tissue may also occur in other body parts of the patient. Especially in men, regional fat deposition as hormone may occur in the body, part, that is, together with gynecomastia, fat in the abdominal region and fat on the sides, and fat on the lower parts of the back. And gynecomastia surgery is usually most commonly combined with abdominal liposuction, sides, and lower back liposuction. In some patients, gynecomastia surgery can also be combined with six-pack surgery, that is, hi-def liposuction surgery, especially in patients who want an athletic appearance of the abdomen. If the regional fat deposition is not too much, if liposuction is not to be performed in a very large area, or if it is in the form of a growth in breast tissue in the form of only a hardness under the nipple. We can perform this surgery under local anesthesia in such patients. But if it is a combined surgery, that is, liposuction to the abdomen, liposuction to the sides, and a six-pack surgery are to performed, or gynecomastia will be used with both vaser liposuction and tissue excision and if the patient has gynecomastia at a level where surgical excision can be performed, then we prefer to perform these surgeries under general anesthesia. After gynecomastia surgery, the patient must definitely wear a corset. The purpose here is for the excess skin to settle well on the muscle tissue, so that there is no gap between the skin and the muscle tissue under it. This gap may cause a fluid accumulation there, such as blood accumulation. Therefore, patients should wear corsets after surgery.
