But can that surgery be done safely? The question that would come up is, "You say that the switches for the hand are next to the tumor. When removing the tumor, will our hands and legs get affected? or will the patient become unable to speak?`` Many such questions are asked by the patients' attenders. 20 years back, that risk was there. Even before 10 years, such a risk was there. In the last 10 years, the advancements in brain surgery have been tremendous. If we look at the advancements, I had discussed the MRI with you. Last time, I had mentioned to you about FMRI, tractography and all. What is FMRI? If there is a tumor, what are the switches around the tumor? When doing the surgery, will the switches be affected? such things will be planned in the MRI itself. So the planning starts not with the surgery, it is even before the surgery. Majority of the time, in the MRI scan stage itself the planning of the surgery is done and other decisions are taken. In the brain, there are the eloquent and non eloquent regions. 80% is non eloquent in the front and in the sides. In most circumstances, they will be non functioning areas of the brain. That is, it would function but it would not be seen obviously. Eloquent regions of the brain are the most important part of the brain; the switch for the hands, legs and speech, switches for the eyes and ears. Such regions where the important nerves are present are called eloquent regions. 20% to 25% of the tumors grow in these areas. That is, one out of five tumors grow in such eloquent regions. Surgery in the non eloquent regions does not affect much. The tumor can be removed completely in a safe way. When removing tumors from the eloquent regions, or tumors that are not on the surface and are very deep, a lot is to be planned and the right gadgets have to be used. What are the advancements for that? I have told you that we do the FMRI first and we can plan using FMRI. For certain parts, for instance, assume we have to go to some place new and we do not know the address. If a certain region is deep, earlier, we cannot dig deep into the brain, in the other parts we can investigate through the layers. In the brain, each cell is important. We cannot explore like that. Therefore in most cases, they would suggest that the patient goes directly for radiation as surgery isn't safe. However, in today's scenario, after there are possibilities of navigation, safely, we can reach any depth. That is, we will fix and load in the navigation machine in the operation theatre. Then correctly, where should we start and at what depth would we reach the tumor, there the device for neuro navigation or brain navigation is extremely useful. Now as you can see in this video, how the navigation is done, how do we make an entry, how do we get there, what would be the distance, have we fully removed the tumor, have we reached the depth, where the tumor is, Have all the sides been covered, all such things, as you see in the video can be figured out with the help of the navigation machine. That is, earlier we used to make a huge hole for the surgery, and now it is not needed. After navigation, it is enough if small holes are made as it would be visible. We will know where the tumor is and there is no necessity to make a large opening. Similarly, earlier we used to completely shave the patient's head before doing the surgery. Nowadays, in most scenarios, the patient's head is not shaved. All the stitches will be under the hair. In most cases, we do not shave their heads. Wherever the stitches are put, there alone, the hair is carefully trimmed, in the operation theatre itself.
