This was a case of a hard cataract with non dilating pupil and a floppy iris. Some useful tips are : 1. The use of 5 iris hooks with 2 just beside the main incision is a good technique to avoid iris injury while introducing and removing the phaco handpiece in the eye. Dont overfill the AC with visco and instead tent up the iris with visco before applying the hooks. 2. A direct vertical chop is a safe and efficient technique since we dont need to go in the periphery blindly. Stay in the center and emulsify nuclear pieces one by one. Use low IOP and low Vacuum and flow rate. 3. Repeated use of good viscoelastic is helpful to protect corneal endothelium since we are working closer to the cornea as AC is very shallow. 4. Make sure to sweep and angle of AC and also go behing the iris to remove all lens matter and visco before closing the eye.
