The ultrasound findings of a dilated main pancreatic duct (5 mm) and common bile duct (8 mm) indicate an obstructive process involving the pancreatic and biliary systems. In the context of a suspected neuroendocrine tumor (NET) of the pancreas, this dilation suggests a possible mass or lesion causing ductal obstruction. Other potential causes include pancreatic adenocarcinoma, especially in the head of the pancreas, which can block both ducts. Tumors of the ampulla of Vater, chronic pancreatitis, or choledocholithiasis (bile duct stones) are also possible. Each of these conditions can impair drainage and result in ductal dilation. Prognosis depends on the underlying cause. Neuroendocrine tumors often have a more favorable prognosis if diagnosed early and resected. However, pancreatic adenocarcinoma typically has a poor prognosis, especially in advanced stages. Further imaging (CT, MRI) and possibly biopsy or endoscopic ultrasound (EUS) are crucial for confirming the diagnosis and guiding treatment.
