As with any surgical procedure, the use a standardized approach will ensure reproducible outcomes. Following a pathway allows the surgeon a template recipe to approach all the variables that one may encounter during surgery. The following video summary series will take the surgeon through the same 9 step format I follow during Greenlight XPS prostate surgery for the treatment of male BPH. In step 5, the pre-established adenoma of the left lateral lobe has been isolated on a stalk to the surgical capsule between 5'- o'clock and 3'-o'clock. As in the previous video of step 4, we have also helped release the lateral adenoma from the apical margin thereby avoiding any sphincteric injury. At this point, the surgeon can freely use the 180W laser setting with delivery of the power into the tissue. Energy delivery to the capsule is thereby minimized. A cleaner defect is the end result of using the higher power and creating an enucleation-like defect.
