Annular pancreas, pancreas divisum, and heterotopic pancreas are congenital anomalies of the pancreas that can have significant clinical implications. In this detailed discussion, Dr. Gunjan Desai, DNB Teaching Faculty for Gastrointestinal, Liver, and Pancreatic Surgery at Lilavati Hospital, Mumbai, explains these conditions, their embryological development, clinical presentations, diagnostic approaches, and management strategies. Annular pancreas is a rare congenital condition where a ring of pancreatic tissue encircles the second part of the duodenum, potentially leading to duodenal obstruction. This anomaly occurs due to the failure of the ventral pancreatic bud to properly migrate during embryogenesis. While some cases remain asymptomatic, others present with symptoms such as vomiting, feeding intolerance, or even pancreatitis. Diagnosis is often made using imaging techniques like CT scans or MRCP, and surgical intervention, such as duodenojejunostomy, may be required in symptomatic cases. Pancreas divisum is the most common congenital variant of pancreatic ductal anatomy, resulting from the failure of the dorsal and ventral pancreatic ducts to fuse during fetal development. This leads to the majority of pancreatic secretions draining through the smaller minor papilla, which can predispose some individuals to recurrent pancreatitis due to inadequate drainage. Although many cases are asymptomatic, symptomatic patients may present with abdominal pain and recurrent pancreatitis. Diagnosis is primarily achieved through MRCP or ERCP, and treatment options range from conservative management to endoscopic or surgical interventions in refractory cases. Heterotopic pancreas, also known as ectopic pancreas, refers to the presence of pancreatic tissue outside its normal anatomical location without direct vascular or anatomical continuity with the main pancreas. It is commonly found in the stomach, duodenum, or jejunum and is usually asymptomatic. However, in some cases, it may cause complications such as inflammation, obstruction, or even neoplastic transformation. Diagnosis is often incidental during imaging or surgery for other conditions, and treatment is typically conservative unless symptomatic, in which case surgical excision may be necessary. Through this educational video, Dr. Gunjan Desai provides in-depth insights into these congenital pancreatic anomalies, their pathophysiology, and the latest advancements in their diagnosis and management. This is a must-watch for medical students, surgical trainees, and healthcare professionals seeking to enhance their understanding of pancreatic disorders.
