Bladder outlet Obstruction | Enlarge Prostate | Brunn's cyst A 41-year-old male who was evaluated for obstructive urinary complaints. Both Kidneys are normal in position, shape and size.( Bipolar length Rt Kid 10.33 cm Lt Kid 10.26 cm ) Cortico-medullary differentiation is normal in both kidneys with normal cortical echogenity. Echogenic renal sinuses are normal on both sides. No evidence of hydronephrosis, mass lesion or calculus is seen on either side. Ureter: No abnormality could be detected in either of the ureter. Urinary Bladder: Well filled with smooth outline. Wall thickness is within normal limit. A cystic mass noted near outlet measuring 1.36cm. Post void residue is about 89ml. Prostate: Enlarge in size. It shows homogenous echotexture with smooth and intact capsule. It measures – Volume: 39 ml. Seminal vesicle: Normal in appearance. Evidence of cystic mass at bladder outlet (1.36cm) The rare case of a Brunn's cyst causing bladder outlet obstruction in a young man. Enlarge prostate (39ml) with increase PVR ( 89ml) Bladder outlet obstruction in younger men is often secondary to urethral stricture disease. In the older population it is often a result of benign prostatic hypertrophy. Ultrasound and cystoscopy revealed a cystic lesion at the bladder neck. Final pathology revealed a cystic structure with predominately denuded urothelium with multiple submucosal cystically dilated von Brunn's nests, most consistent with a Brunn's cyst.
