This procedure video illustrates use of a columella retraction suture to obliterate dead space after correction of the nasolabial and columella-tip complex in rhinoplasty. Precise palpation of the columella in a posterior direction with the tip of a blunt instrument makes judgment of the existence of dead space and the most desirable position of retraction (taking into account double break and nasolabial angle) possible. This position is marked with surgical ink. A Vicryl 4-0 with straight SC1 needle is entered from the vestibular side, cephalic to the footplate when no narrowing of the columella is needed, or through the footplate when the surgeon feels the need to bring the footplates closer together. The needle exits the columella at the ink mark. This exact puncture hole is reentered with the same needle making sure the suture will lie under the skin. The needle is pushed out on the other vestibular side (through or behind the footplate) and then passed back through the septum 2-3 mm more cephalic from 1 vestibulum to the other. It is tied, making sure the approximation of the footplates and retraction of the columella achieves the desired result. The suture is cut and/or replaced in cases of any unsatisfactory.
