It is essential to distinguish this: vesiculobullous lesions are not expected in adult secondary syphilis and their presence should point toward congenital infection. Findings Commonly Seen in Secondary Syphilis: Numerous lesions covered with easily removable scales: Seen as coin- or discoid-shaped psoriasiform lesions with serous discharge (scales are easily removed). Moist, well-demarcated papules and plaques: Condyloma lata, very characteristic of secondary syphilis, appear as moist, flat-topped lesions in genital/anal areas (not to be confused with condyloma acuminata, which is due to HPV). Serpiginous, non-ulcerative lesions of oral mucosa: Mucous patches (sometimes described as snail-track) are common; though the word "ulcer" is misleading, these are not true ulcers, just mucous membrane patches. Other Points: Secondary syphilis can also show a non-scarring, patchy scalp hair loss called "alopecia syphilitica." The earliest rash is macular, also called "roseola syphiliticum," which can leave a hypopigmented ring (collarette)—termed "necklace of Venus." Summary: Vesiculobullous eruptions are not seen in secondary syphilis and are a feature of early congenital syphilis. Typical findings in secondary syphilis include psoriasiform scaly lesions, condyloma lata, and serpiginous mucous patches in the oral cavity. Recognizing these distinctions is key for exam questions and clinical practice.
