This 46-year female presented with 7 days history of rotational vertigo on getting up from supine, lateral recumbent to right, bending the trunk forward and this was associated with a constant feeling of disequilibrium. Recently she had been diagnosed type-2 Diabetes Mellitus about a month back and was taking metformin 500 mg twice a day for this. Her Dix-Hallpike Test to either side as well as in the enhanced head-hanging position elicited a downbeating nystagmus with left torsional component, and the later was most prominently seen during the right Dix-Hallpike and enhanced straight head-hanging positionings. A diagnosis of right apogeotropic posterior semicircular canal BPPV (non-ampullary right posterior semicircular canalolithiasis) is considered and she is treated with Quick Liberatory Rotation Maneuver described by Luigi Califano (2003, 2014 and 2021) for three times. After 2 hours she was assessed by right Dix-Hallpike test and this did not elicit any positional nystagmus and accompanying vertigo.
