Postoperative capsule opacification (PCO) is the most common postoperative complication of cataract surgery. Following the migration and proliferation of the lens's remaining epithelial cells, the posterior capsule becomes opacified. Laser capsulotomy is an efficient PCO treatment. Although it has not yet been proven in trials, many surgeons advise removing the majority of the debris, particularly when it involves the central visual axis. There are several methods for polishing the posterior capsule, all of which should be done carefully after weighing the risks and benefits. To polish the posterior capsule, I usually use a 27-gauge blunt cannula attached to a silicone sleeve. However, in some cases where the risk of posterior capsule tear or zonular dehiscence is higher, or when a premium lens should be implanted, I prefer to perform the laser capsulotomy later instead of polishing the posterior capsule.
