Loosening and Instability Jikku Haniball Failing total knee arthroplasty is a common issue, which can be classified into three groups: structural, biologic, and kinematic. The revision risk following primary TKA at ten years follow-up is 5 percent, and 45 percent range of failure is reported in most countries. Aseptic loosening is the most common indication for revision surgery followed by infection. A diagnostic algorithm for the evaluation of failed total arthroplasty was proposed by Kelly Wiens in 2014, which includes eight modes of failure. The diagnosis of failing knee replacement requires a thorough examination, including range of motion, stability, patella tracking, and radiographic evaluation. Extra-articular causes of pain should also be considered.
