If excess fluid is around, it can produce a condition called cardiac tamponede, that is it is compressing the heart. This is an emergency condition where you have to immediately relieve by putting a catheter and a needle into the pericardial space and aspirate it , what you call as pericardiocentesis or we have to make an open window and drain those fluids by putting an intercostal drainage tube there. Basically the fluid has to be removed and the pericardium has to be removed. That is the surgery for pericarditis.
