This is a case shared by Dr. Ricardo Leitão Guerra from Salvador Brazil. This is a complex case of Traction retinal detachment secondary to Diabetic retinopathy. In the beginning of the surgery, after removing the vitreous core, it's important to release anterior-posterior traction that connects the periphery and the posterior pole. Start in a place where the posterior hyaloid is detached in the midperiphery and perform a circumferential rexis trying to remove the most you can of the vitreous detached from the retina. All the adherent fibrovascular membranes are left to be dissected in the next step. To remove the fibrovascular membranes you need to find a plane and perform segmentation and delamination. The surgeon in this case used retinal forceps to create a plane from the optic disc to the periphery using the inside out technique.
