Ultrasound Findings A 6 x 4 cm thyroid cyst containing fluid with colloid particles is noted. Within the cyst, there is a 1.0 x 1.5 cm mural hypoechoic solid nodule with smooth margins, minute comet-tail echogenic foci, and marked internal vascularity. TIRADS Score The nodule scores as follows: Composition (mixed cystic and solid): 1 point Echogenicity (hypoechoic): 2 points Shape (wider-than-tall): 0 points Margin (smooth): 0 points Echogenic foci (comet-tail artifacts): 0 points **Total TIRADS points: 3 (TR3 - mildly suspicious) Prognosis TR3 nodules have a 5% to 10% risk of malignancy. The presence of marked internal vascularity increases clinical suspicion. Management For TR3 nodules, fine-needle aspiration (FNA) is considered if the nodule is ≥2.5 cm. Given the vascularity and clinical context, FNA is recommended for this 1.0 x 1.5 cm nodule. If FNA is deferred, close ultrasound follow-up is advised.
