His initial labs were grossly normal with normal kidney function (creatinine of 1.1) and a normal creatinine kinase, but a slight elevation in his troponin to 37. Troponin peaked 1385 the evening of his admission, before downtrending the following day to 430. There were no arrhythmias identified on his baseline and follow-up electrocardiograms, which were normal sinus rhythm, and resuscitation was unremarkable. After demarcation of his burns, the patient was taken to the operating room on hospital day four where he underwent an uncomplicated wound bed preparation and placement of a dermal substitute. His plethora facial fractures spared the pterygoid plates leading to a LeFort type II and III pattern of facial fractures without true maxillo-mandibular dissociation, with almost full resolution of soft-tissue swelling and pain by hospital day three.
