A PE is a potentially life-threatening medical emergency caused by a blockage in a lung artery, usually by a blood clot (embolus) that traveled from the legs or pelvis. This blockage reduces blood flow and oxygen to the lungs, leading to symptoms like shortness of breath and chest pain. Treatment focuses on dissolving the clot and preventing new ones from forming. In this essential video, I break down the systematic approach to risk-stratification, focusing on how we quickly determine a patient's acuity and guide treatment. The Risk Spectrum: Understanding the critical distinction between low, medium, and high-acuity PE. Low Risk: The patient is hemodynamically stable, and there are typically no concerns of Right Ventricular (RV) strain. High Risk: This is a critical state, implying hemodynamic compromise such as hypotension or concerns for obstructive shock, which requires immediate and aggressive intervention. The PESI Score: I go over the fundamentals of the Pulmonary Embolism Severity Index (PESI) score, a vital clinical tool used to determine the basics of a patient's PE management plan.
