Multiple gaps were found in how various agencies in Singapore had handled the case of four-year-old Megan Khung, who died in February 2020 from child abuse. This is the conclusion by a review panel, tasked by the Social and Family Development Ministry to look into their actions, protocols and processes. Following the review, two police officers were disciplined for not following protocols. A child protection officer from the ministry is also going through a disciplinary investigation. Ms Pang Kee Tai, Deputy CEO at PAVE Integrated Services, talks about the biggest lessons that child protection practitioners should take away from this tragedy.
