video transcript - revision rhinoplasty surgery is a lot more complex than a primary procedure first of all I often have patience to come and see me who we really don't know what they've had done before in terms of the actual surgical process it's nice to have previous operative notes so that we can check but sometimes these are incomplete or really not it reflective of what was done before and it's always difficult treating revision patients they've been through an operation before they may have put felt quite traumatized by that and they can be emotional issues associated with it as well so we really get to know our patients we left spend a lot of time going through what's important for them to try and correct and really try to engage with them to make sure that what we achieve second time round is as close to what they're able to do now of course they are challenges in revision rhinoplasty in the sense that there may be things that were done first time that are not correctable but will we use a variety of methods and this is really where experiences the key you cannot do revision surgery and unless you've done you have done loads and loads of these sorts of procedures and very importantly have a variety of techniques available to you I can plan operations all the time and we produce a surgical plan before the operations but unfortunately this doesn't always pan out because when we open the nose and we see what's going on inside it's not quite as we expected so expecting the unexpected is part of what up what we do and making sure that patients understand what the limitations are and hopefully this sort of results we can achieve for them will ensure that we get happy patients at the end of it it is complex and it can be very tiresome and we spent a lot of time planning these operations to make sure that we really get the best result for our patients we often need to take cartilage as part of a revision procedure this may be because during the original operation and they've already taken cartilage and from the nasal septum which is the primary preference from my point of view obviously if we take cartridge from within the nose itself that's excellent and sometimes that college is not available in which case we'll go to the ear is almost being a spare tire for the nose and providing it a source of cartridge and when that isn't enough we can even use cartilage from the rib which is almost the tire shop because it's plentiful and this can be carved in a variety of different ways wherever possible we try not to use any kind of implant in there for the reason that implants can be problematic in terms if infection however they can occasionally be useful but we would discuss with patients on a skates t case basis
