Basic Life Support (BLS) Tips on passing CPR training in 2021. BLS or Basic Life Support Tips. BLS is important for nursing and general public. You want to: A) Ensure environment safety B) Move to patient and identify if they are responsive. C) Identify if there are others to help you: call 911, give ventilations, assist you doing chest compressions, and obtaining AED D) If they are unresponsive; check for carotid or radial pulse. E) If no Pulse: § Call for help § Send someone for AED or equipment available (crash cart) and place it on the patient once you have it on. § Start compressions right away. § After 2 minutes of compressions do a pulse check. § When help arrives assess the need for LMA or OPA/NPA (do you need to protect their airway?) Automatic External Defibrillator (AED) Review ○ Follow AED prompts always. ○ Let it anaylze rhythm - never do CPR ○ Resume compressions while defibrillator charges ○ Clear client to administer the appropriate shock ○ Immediately resume compressions post shock ○ Make sure you shave chest for the AED. ○ Snow or small puddle, we can use AED. If the patient is wet, dry them. ○ If the patient has a pacemaker, do not put pad over it. Place it underneath. ○ Take medicine pad off if it is in the spot of the AED pad. ○ If the patient is 8 years or older use adult pads, if unsure use adult pads. What does a shock do? The shock temporarily “stuns” the heart muscle. This stops the VF or pVT and “resets” the electrical system of the heart, so a normal (organized) heart rhythm can return. If an organized rhythm returns and high-quality CPR continues, the heart muscle can begin to contract and pump blood effectively. Teach you how to properly provide chest compressions: § 30 chest compressions to 2 ventilations § 100 - 120 beats per minute § Allow for full recoil § 30:2 for 2 minutes and re-check pulse. Pulse checks: § Check carotid or radial pulse for 10 seconds MAXIMUM § If remains pulseless immediately start CPR Cardiac arrest: Electrical malfunction in heart that causes an irregular heart beat. This causes inadequate blood supply to be given to vital organs in the body such as the brain, and lungs. Decreased blood supply causes cell death and ultimately leads to the organ not functioning anymore. What to do? § Call 911 § Start CPR right away § Obtain AED § If 2 people available; 1 call 911 and the other grab AED. Agonal Gasps: ○ Not normal breathes. ○ Present in the first few minutes after sudden cardiac arrest. ○ Gasping is a sign of cardiac arrest - it is not normal breathing. ○ Gasps may sound like snort, snore, groan. Jaw Thrust: Jaw thrust maneouvre is used when head-tilt/chin lift doesn’t work or a spinal injury is suspected. 1) Place hand on each side of victims head. 2) Place fingers under angles of lower jaw, displacing the jaw forward. 3) If the lips close, push the lower lip with your thumb to open the lips. Ventilations: ○ Avoid excessive ventilation. Pause about 1 second between each breath. ○ Look for chest rise when giving ventilations ○ Ensure proper securement of airway ventilation device by properly sealing the mouth. Important concepts: ○ Closed-loop communication is important for team leader and team members. § Team leader: call each team member by name and make eye contact. Do not assign additional tasks until you are sure that the team member understands the instruction. § Team member: confirm that you understand by verbally acknowledging. Tell the team leader when you have finished a task. Child versus Adult BLS. ○ Regardless of age of the victim you need to compress at 100-120/minute Immediately activate emergency response system. If the patient was unconscious when the healthcare professional arrived, then immediately start doing CPR for 5 cycles. If it was a witnessed unconscious effort; immediately activate the emergency response team. Children or infants with no pulse or pulse less than 60 beats per minute with signs of poor perfusion despite adequate oxygenation, CPR should be started.
