Reporting medical errors: Top tactics for improvement

Researchers at St. Jude Children's Research Hospital set out to understand workplace factors that influence voluntary reporting of medical errors and patient safety events. They analyzed data from a survey that measures 10 factors that reflect and influence the culture of patient safety, both hospital-wide and at the department level. The survey was from more than 220,000 health care professionals at almost 1,000 hospitals. The analysis showed that providing feedback to staff who report patient safety events was the cultural factor most associated with voluntary reporting of errors. Feedback was important regardless of the severity of the mistake involved.
