Surgical difficulties faced in a Laparoscopic Cholecystectomy - Dr. Nanda Rajaneesh

The Laparoscopic Cholecystectomy is one of the standard of care for removal of gall bladder. This is one of the first laparoscopic surgeries introduced in general surgery and lot much things have been changed once surgical procedure has been introduced. The gall bladder is an organ sitting below the liver at the depth in the upper abdomen. To cut the abdomen and go by open surgery it is difficult. So we go by Laparoscopic surgery. So the problems that we can encounter is only when the gall bladder is situated in abnormal position where it is buried in the liver or it is completely buried with the caudate lobe overlapping it, simple gall bladder also there can be difficulties. But most of the difficulties in laparoscopy comes when there is adhesion infection, recurrence of cholecystitis that would have happened in that area, empyema that is pus collected in the gall bladder area and surrounding tissues coming and attaching over there. Infection of the duodenum, stones slipping done into the common bile duct and eroding the common bile duct which is called as Mirizzi's syndrome. So in these situations we do encounter lot of difficulties in dissection and in cutting the tissues there are cancers that there are bleeding because we might cut the arteries because the visibility is poor in a situation where it starts bleeding an there can be tissue injuries like duodenal injuries, common bile duct injuries when there is an infection. So these are the difficulties that we can face only in difficult gall bladder. So we use the terminology difficult gall bladder. Simple gall bladder mostly the problems we encounter is very less.
