ACL injuries commonly occur in adolescents from ball sports that have a lot of side-to-side motion and cutting, such as soccer. As young people's bodies grow, their bodies can't always end up handling those motions and they can tear the anterior cruciate ligament (ACL). Females are at higher risk for ACL tears than males. The best treatment for youth experiencing ACL tears is reconstruction, non-operative treatment with braces and exercise is not typically successful. The ligament can not repair itself like a broken bone. A tendon is usually taken from another part of the body and used to rebuild the torn ligament. In the past, surgery was delayed when possible and orthopedic surgeons typically used a hamstring graft. Today, the best practice is to use a quadriceps graft. The re-rupture rates are lower and kids are able to return to activities more quickly. Physical therapy is a critical component to allow children to return to sports after an ACL tear. The orthopedic surgeon can give the child a high-quality graft and functional knee, but the child needs to work hard to strengthen the muscles around the knee and regain the control of the knee. Dr. Justin Roth, pediatric orthopedic surgeon at The Center, recommends nine months of PT at a minimum, with rechecks every six months as the child continues to grow. Re-injury rates amongst adolescents for ACL tears are substantially higher than in the general population.
