Cutaneous horns are due to thick layers of keratin built up over the surface of keratinocytic epidermal lesions, most often verruca vulgaris, actinic keratosis, or squamous cell carcinoma. Biopsy and microscopic examination by a pathologist is the only way to know which lesion is causing the horn. Some horns can become huge and quite dramatic clinically. This video explains.
