Presented by the Association of Clinicians for the Underserved on September 15, 2020. This workshop will serve a brief training in the Collaborative Care Model, demonstrating what makes it a higher level of care for patients through the integration of behavioral health services into primary care and physical health settings. Education on team structure, workflows, and evidence base for the model will be presented. Presenter and participants will explore the clinical benefits of Collaborative Care (CoCM) and the evidence of improved health outcomes for patients receiving Collaborative Care. It will briefly introduce the system changes needed to implement this model (EHR, registries, provider engagement, and metrics) and demonstrate the financial benefits of the model for practices (billing codes, reimbursement for Medicare beneficiaries and other reimbursement opportunities, and future plans for increased reimbursement across the country). Interactive components will include case study examples, a look into workflows, and success stories of practices implementing the Collaborative Care Model.
