30 year old patient with cataract secondary to steroid use for myasthenia gravis. In these young patients where the eventual need for a YAG would otherwise be a certainty, I advocate for a PRIMARY POSTERIOR continuous curvilinear capsularexis. This patient is non-vitrectomized. But with care, the anterior hyaloid is never violated and vitrectomy is NOT required. Extensive polishing of lens epithelial cells (LECs) will decrease fibrosis and sommerings formation as the decades go by. Should the patient ever need or want exchange, it will be so much easier to execute, and she will remain a candidate for single piece lenses. Note, had the patient been even younger (especially if a child) I would have also performed posterior optic capture to eliminate the risk of residual cells migrating across the optic. I could have done that here too, but wanted to decrease the apposition and scarring of the anterior and posterior capsules together... or at least make it overall easier to later reinflate the capsular bag
