Endoscopic treatment of pathological lymphatic leakage after Fontan procedure (3D) - ESPGHAN Meeting

The Fontan procedure is a staged palliative surgery for pediatric patients with complex congenital heart defects involving a single functional systemic ventricle. Following the procedure, systemic venous return bypasses the non-functional hypoplastic ventricle via the Glenn and Fontan cavopulmonary anastomoses, leading to chronic venous congestion. Elevated central venous pressure impairs lymphatic drainage through the thoracic duct, initiating lymphatic vessel dilation, lymphangiectasia, and the formation of abnormal lymphatic collaterals, particularly in the hepato-duodenal region. In the intestine, these changes cause dilation and wall thinning of lacteals, leading to leakage of protein-rich lymph into the intestinal lumen. Clinically, this manifests as protein-losing enteropathy, with hypoalbuminemia, edema, and malnutrition. Management requires a multidisciplinary approach.
